The CORE team serves as an entry point into the County’s coordinated entry system for unsheltered persons and work to locate, engage, stabilize and house chronically homeless individuals and families. The CORE Program provides homeless outreach services aimed at identifying homeless individuals, youth and families living without shelter and in locations not meant for human habitations. The County has been providing homeless outreach services under this contract since August 2017.
On July 24, 2018, the Board of Supervisors approved Agreement #29-812-1 to receive funds in an amount of $13,000 from the City of Concord to provide homeless outreach services for the period July 1, 2018 through June 30, 2019.
On July 30, 2019 the Board of Supervisors approved Agreement #29-812-2 to increase the amount payable to the County by $13,000 to a new amount not to exceed $26,000 from the City of Concord and to extend the termination date from June 30, 2019 to June 30, 2020.
On June 2, 2020, the Board of Supervisors approved Agreement #29-812-3 to increase the amount payable to the County by $25,000 to a new amount not to exceed $51,000 with no change in the term July 1, 2018 to June 30, 2020.
On October 13, 2020, the Board of Supervisors approved Agreement #29-812-4 to increase the amount payable to the County by $13,000 to a new amount not to exceed $64,000 and to extend the termination date from June 30, 2020 to June 30, 2021.
The County received the documents for this agreement from City of Concord on August 11, 2021.
Approval of Agreement #29-812-5 will allow the County to continue to receive Concord/Pleasant Hill Health Care District funds for operation of the CORE Program through July 31, 2022. This agreement includes agreeing to indemnify and hold harmless the contractor for claims arising out of the County’s performance under this contract.
If this agreement is not approved, the County will not receive funding and without such funding, the CORE program may have to operate at a reduced capacity.
