RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County, Contract Amendment Agreement #77-008-2 with Mission Hills Eye Center, a corporation, effective September 1, 2018, to amend Contract #77-008-1, to increase the payment limit by $400,000, from $600,000 to a new payment limit of $1,000,000 with no change in the original term of January 1, 2018 through December 31, 2019.

FISCAL IMPACT:

This amendment is funded by 100% by Contra Costa Health Plan (CCHP) Enterprise Fund II. (No rate increase)

BACKGROUND:

On January 9, 2018, the Board of Supervisors approved Contract #77-008-1 with Mission Hills Eye Center to provide ophthalmology and optometry services to CCHP members for the period January 1, 2018 through December 31, 2019.

BACKGROUND: (CONT'D)

Approval of Contract Amendment Agreement #77-008-2 will allow the contractor to provide additional ophthalmology and optometry services for CCHP members through December 31, 2019.

CONSEQUENCE OF NEGATIVE ACTION:

If this amendment is not approved, CCHP Members will not receive the benefits of ophthalmology and optometry services from this contractor.
