RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Grant Agreement #29-685 from University of California, San Francisco (UCSF), to pay the County an amount not to exceed $32,000 including modifications to the County Standard Indemnification clause, to create the Connection to Health for Smokers (CTHS) Program to help smokers quit and improve their general health, for the period from September 1, 2018 through August 31, 2020.

FISCAL IMPACT:

Approval of this agreement will allow the County to receive an amount not to exceed $32,000 from UCSF to support the creation of CTHS Program through August 31, 2020. (No County match required.)

BACKGROUND:

UCSF’s Connection to Health (CTH) is a comprehensive electronically supported program to facilitate patient assessment, priority setting, action planning and follow-up to improve health. The CTHS Program is an integrated module within CTH that incorporates dedicated and enhanced behavioral change tools and strategies to support smoking reduction and cessation.  
  
Approval of Grant Agreement #29-685 will allow the County to receive support for the CTHS Program through August 31, 2020.

CONSEQUENCE OF NEGATIVE ACTION:

If this agreement is not approved, the County will not be able to receive funding for the CTHS Program.
