RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #77-159-4 with Pathways Home Health and Hospice, a non-profit corporation, in an amount not to exceed $300,000, to provide home health care and hospice services for Contra Costa Health Plan (CCHP) members, for the period from August 1, 2022 through July 31, 2025.

FISCAL IMPACT:

This contract will result in contractual service expenditures of up to $300,000 over a three-year period and will be funded 100% by CCHP Enterprise Fund II allocations. (No rate increase)

BACKGROUND:

CCHP has an obligation to provide certain specialized home health care and hospice services including, but not limited to: registered nurse and infusion visits, certified home health care aide, medical social worker visits, speech and physical therapy visits according to the terms of their Individual and Group Health Plan membership contracts with the County. This contractor has been a part of the CCHP Provider Network since August 1, 2018.

BACKGROUND: (CONT'D)

In November 2020, the County Administrator approved and the Purchasing Services Manager executed Contract #77-159-2 in the amount of $150,000 for the provision of hospice care services for CCHP members for the period August 1, 2020 through July 31, 2022.

In March 2021, the County Administrator approved and the Purchasing Services Manager executed Contract Amendment Agreement #77-159-3, effective March 1, 2021 to increase the payment limit by $50,000 to and new payment limit of $200,000 to include home health services in addition to hospice services for CCHP members with no change in the original term of August 1, 2020 through July 31, 2022.

Approval of Contract #77-159-4 will allow this contractor to continue providing home health care and hospice services to CCHP members through July 31, 2025.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialized home health care and hospice services for CCHP members under the terms of their Individual and Group Health Plan membership contracts with the County will not be provided.
