Acceptance of the grant agreement will result in payment to the County of up to $157,665 in funding for the 15-month grant from the CDPH. No County match is required.
The Health Services Department’s Public Health Division’s Family, Maternal, and Child Health NFP Program works by having specially trained nurses regularly visit young, first-time moms-to-be, starting early in the pregnancy, continuing through the child’s second birthday. Through individualized goal planning, life coaching, and education, NFP nurses support our mothers to successfully transform their lives and that of future generations.
As designated by the CHVP ARP, funds can be used toward providing prepaid grocery cards, in the amount of $200 each, to the Department's Maternal Infant Early Childhood Home Visiting NFP clients for the purpose of meeting the emergency needs of the families. The department expects to purchase up to 788 gift cards from Walmart. Each client will receive 1 card upon completion of program family assessment timepoint periods: 1) 5-7th pregnancy encounter, 2) 36th week gestational age encounter, 3) infant birth encounter, 4) infant 6 month encounter, 5) toddler 3rd visit encounter, 6) toddler 18 month encounter, 7) toddler 24 month encounter.
Approval of Grant Agreement #78-001 will allow the Health Services Department to provide Walmart and grocery gift cards to NFP clients to assist with emergency food needs of families, through September 30, 2023. This agreement includes the County’s agreement to indemnify and hold harmless the Grantor for claims arising out of County’s performance under this contract.
