RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #76-648 with Ambassadors of Health Management, LLC, a limited liability company, in an amount not to exceed $109,028, to provide residential board and care services for Contra Costa Regional Medical Center (CCRMC) and Contra Costa Health Center patients in the Patch Program, for the period July 1, 2019 through June 30, 2020.

FISCAL IMPACT:

This Contract is funded 100% by County Patch Program Funds. (No rate increase)

BACKGROUND:

The County’s Patch Program provides residential board and care for post medical, surgical and/or custodial care patients who have been discharged from CCRMC and would otherwise not have appropriate follow up care.

Under Contract #76-648, Contractor will provide residential board and care services for CCRMC and Contra Costa Health Center patients in the Patch Program, for the period July 1, 2019 through June 30, 2020.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, these post-surgery patients would not have access to Contractor’s services.
