RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County, Contract #77-165 with Guardian Home Health Care & Hospice, Inc., a corporation, in an amount not to exceed $300,000, to provide home health and hospice services to Contra Costa Health Plan (CCHP) members for the period July 1, 2018 through June 30, 2020.

FISCAL IMPACT:

This contract is funded 100% by CCHP Enterprise Fund II.

BACKGROUND:

Under Contract #77-165, the contractor will provide home health and hospice services for CCHP members for the period July 1, 2018 through June 30, 2020.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialty health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the County will not be provided.
