RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County, Agreement #29-812-1 with Concord/Pleasant Hill Health Care District, a subsidiary of the City of Concord, to pay the County an amount not to exceed $13,000, for the operation of the Coordinated Outreach, Referral and Engagement (CORE) Program, for the period July 1, 2018 through June 30, 2019.

FISCAL IMPACT:

Approval of this agreement will allow the County to receive an amount not to exceed $13,000 from the Concord/Pleasant Hill Health Care District, a subsidiary of the City of Concord, to provide homeless outreach services. No County funds required.

BACKGROUND:

On September 12, 2017, the Board of Supervisors approved Agreement #29-812 to receive funds from the City of Concord for the provision of the CORE Program, which provides homeless outreach services aimed at identifying homeless individuals, youth and families living without shelter and in locations not meant for human habitation.

Approval of Agreement #29-812-1 will allow County to receive Concord/Pleasant Hill Health Care District funds to operate the CORE Program through June 30, 2019. This agreement includes agreeing to indemnify and hold harmless the contractor for claims arising out of County’s performance under this contract.

CONSEQUENCE OF NEGATIVE ACTION:

If this agreement is not approved, County will not receive funding and without such funding, the CORE program may have to operate at a reduced capacity.
