RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Service Director, or designee, to execute on behalf of the County Contract #77-224-1 with Familytree Medical Transport, LLC., a limited liability company, in an amount not to exceed $400,000, to provide non-emergency medical transportation services for Contra Costa Health Plan (CCHP) Members, for the period from August 1, 2020 through July 31, 2021.

FISCAL IMPACT:

This Contract is funded 100% by CCHP Enterprise Fund II. (No rate increase)

BACKGROUND:

On April 9, 2019, the Board of Supervisors approved Contract #77-224 with Familytree Medical Transport, LLC for the provision of non-emergency medical transportation services for CCHP Members for the period June 1, 2019 through July 31, 2020.

BACKGROUND: (CONT'D)

Approval of Contract #77-224-1 will allow the Contractor to continue to provide non-emergency transport services for CCHP Members through July 31, 2021.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain services for CCHP members will not be provided.
