RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #27-939-5 with Axis Community Health, Inc. (dba Axis Community Health), a non-profit corporation, in an amount not to exceed $300,000, to provide primary care services for the period from July 1, 2020 through June 30, 2022.

FISCAL IMPACT:

This Contract is funded 100% by Contra Costa Health Plan Enterprise Fund II. (Rate Increase)

BACKGROUND:

On July 24, 2018, the Board of Supervisors approved Contract #27-939-2, as amended by Amendment Agreement #27-939-3, with Axis Community Health, Inc., for the provision of primary care services to Contra Costa Health Plan members, for the period from July 1, 2018 through June 30, 2020.

BACKGROUND: (CONT'D)

Approval of Contract #27-939-5 will allow the Contractor to continue to provide primary care services through June 30, 2022.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialized professional health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the County will not be provided.
