RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Amendment Agreement #29-812-3 with the City of Concord, to amend Grant Agreement #29-812-1, as amended by Amendment Agreement #29-812-2, to increase the amount payable to the County by $25,000, from $26,000 to a new amount of $51,000, with no change in the term of July 1, 2018 through June 30, 2020

FISCAL IMPACT:

Approval of this amendment agreement will allow the County to receive an additional amount not to exceed $25,000 for Fiscal Year 2019/2020 from the City of Concord. No additional County funds required.

BACKGROUND:

The Coordinated Outreach, Referral and Engagement (CORE) Program provides homeless outreach services to residents in Concord and Pleasant Hill. CORE Program services locate and engage homeless clients throughout Contra Costa County. CORE teams serve as an entry point into the County’s coordinated entry system for unsheltered persons and work to locate, engage, stabilize and house chronically homeless individuals and families.  
  
On July 24, 2018, the Board of Supervisors approved Agreement #29-812-1, as amended by Amendment Agreement #29-812-2, to receive funds from the City of Concord for the provision of the CORE Program, for the period from July 1, 2018 through June 30, 2020. This Agreement included agreeing to indemnify and hold harmless the Contractor for claims arising out of County’s performance under this Contract.  
  
Approval of Agreement #29-812-3 will allow County to receive additional funds for homeless outreach services through June 30, 2020.

CONSEQUENCE OF NEGATIVE ACTION:

If this agreement is not approved, County will not receive funding and without such funding, the CORE program may have to operate at a reduced capacity.
