RECOMMENDATION(S):

ADOPT Position Adjustment Resolution No. 25597 to add one (1) Social Worker (XOVC) (represented) position at Salary Plan and Grade 255 1434 ($5,463.96-$6,641.47) and cancel one (1) Social Service Employment Placement Counselor (X7WB) (represented) position number 15840 at Salary Plan and Grade 255 1432 ($5,271.80-$6,407.90) in the Employment and Human Services Department.  

FISCAL IMPACT:

Upon approval, this action will have an approximate three month cost increase of $1,192. Annual cost increase is estimated at $4,764, with an annual pension cost increase of $886. This cost will be completely offset by revenue from the Whole Person Care (WPC) Pilot Program funds awarded by the California Department of Health Care Services (DHCS) to the Contra Costa County Health Services (CCHS) Department. The positions are funded 100% State revenue.


BACKGROUND:

In July 2017, Employment and Human Services (EHSD) entered an Interdepartmental Services Agreement with Health Services to partner in the staffing the Whole Person Care CommunityConnect Pilot Program.   
  
The overall goal of the program is to increase coordination of health, behavioral health, and social services for Medi-Cal recipients who are at high risk of utilizing high acuity medical services across multiple delivery systems. The program integrates physical health, behavioral health, and social services in a patient-centered manner in order to improve the health and well-being of this vulnerable population. Originally, EHSD requested and was approved to hire six (6) Social Worker and six (6) Social Worker II positions to perform case management services to Whole Person Care patients. In the first round of hiring, three (3) staff in the classification of Social Service Employment Counselor were hired into three of the Social Worker positions. Since this time, one Social Service Employment Placement Counselor position has been vacated. Upon review, the Social Worker position is the preferred position that is needed to meet the needs of Whole Person Care and thus this request for trade is being made.  
  

CONSEQUENCE OF NEGATIVE ACTION:

If this action is not approved, EHSD will not be able to provide the required staff needed to participate in this program and CCHS will be forced to explore other alternatives for social service case management and care coordination potentially risking successful pilot program implementation.
