RECOMMENDATION(S):

Acknowledge receipt of notice from Gastroenterology Associates of the East Bay Medical Group, Inc., a corporation, requesting termination of Contract #27-992, effective at the end of business on April 16, 2019.

FISCAL IMPACT:

This Contract is funded 100% by CCHP Enterprise Fund II.

BACKGROUND:

On October 17, 2017, the Board of Supervisors approved Contract #27-992 with Gastroenterology Associates of the East Bay Medical Group, Inc., for the provision of gastroenterology services for Contra Costa Health Plan (CCHP) members and County recipients, for the period from October 1, 2017 through September 30, 2019.

BACKGROUND: (CONT'D)

The Department received a letter from the Contractor, requesting termination of this Contract. In accordance with General Conditions, paragraph 5 (Termination), the Contractor served the Department 30 days written notice of cancellation of this contract.
