RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County, Contract #23-573-4 with Center for Human Development, a non-profit corporation, in an amount not to exceed $1,040,194, to provide social need resource linkage, prison and jail reentry support services, health education and community outreach services to patients and residents of Contra Costa County, for the period July 1, 2018 through June 30, 2019.

FISCAL IMPACT:

This contract is funded 19% by Medi-Cal Administration Activities and 81% by Health Services Ambulatory Care. (Rate increase)

BACKGROUND:

On June 6, 2017, the Board of Supervisors approved Contract #23-573-3 with Center for Human Development, to provide social need resource linkage, prison and jail reentry support services, health education and community outreach services to patients and residents of Contra Costa County, for the period July 1, 2017 through June 30, 2018.

Approval of Contract #23-573-4 will allow the contractor to continue providing these services through June 30, 2019.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, vulnerable patient populations served in the County health centers and who are residents of Contra Costa will not receive these services from the contractor.

CHILDREN'S IMPACT STATEMENT:

This program supports the following Board of Supervisors’ community outcomes: “Communities that are Safe and Provide a High Quality of Life for Children and Families”. Expected program outcomes include a decrease in reentrant recidivism, a decrease in food insecurity, an increase in patient’s access to Medi-Cal program, health improvements for children with pediatric obesity, adults with diabetes and hypertension and women who are pregnant.
