RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County, Contract Amendment Agreement #24-681-94(1) with PH Senior Care, LLC (dba Pleasant Hill Manor), a non-profit corporation, effective June 1, 2018, to amend Contract #24-681-94, to increase the payment limit by $15,000, from $753,100 to a new payment limit of $768,100, with no change in the original term of November 28, 2017 through November 30, 2018.

FISCAL IMPACT:

This amendment is funded 88% by Mental Health Realignment and 12% by Mental Health Services Act. (No rate increase)

BACKGROUND:

On February 27, 2018, the Board of Supervisors approved Contract #24-681-94 with PH Senior Care, LLC (dba Pleasant Hill Manor), for the provision of augmented residential board and care services, including room, board, care and supervision, for the period from November 28, 2017 through November 30, 2018.

BACKGROUND: (CONT'D)

Approval of Contract Amendment Agreement #24-681-94(1) will allow the contractor to provide additional residential board and care services to a client through November 30, 2018.

CONSEQUENCE OF NEGATIVE ACTION:

If this amendment is not approved, an additional client will not receive services for County’s mentally disordered residents and will not receive the care and supervision needed which may result in increased hospitalizations.
