RECOMMENDATION(S):

ACKNOWLEDGE and APPROVE the termination of the Health Services Department Contract #27-986-2 with Sonja Robinson (dba Healthcare Solutions USA), effective at the close of business on May 25, 2018.

FISCAL IMPACT:

This contract was funded 100% by CCHP Enterprise Fund II.

BACKGROUND:

On November 7, 2017, the Board of Supervisors approved Contract #27-986-2 with Sonja Robinson (dba Healthcare Solutions USA) for the period from December 1, 2017 through November 30, 2018.

In accordance with Special Conditions Paragraph 4.d. (Termination re: Credentialing) and General Conditions Paragraph 5 (Termination and Cancellation) of the Contract, the Health Services Department issued written notice to the contractor immediately terminating Contract #27-986-2 effective at the close of business on May 25, 2018.
