RECOMMENDATION(S):

APPROVE clarification of Board action of January 9, 2018 (Item C.71), to reflect the correct name of the contracting party as Pinnacle Mental Wellness Group, rather than Monica L. Wood (dba Pinnacle Mental Wellness Group).

FISCAL IMPACT:

There is no fiscal impact with this clarification action.

BACKGROUND:

On January 9, 2018, the Board of Supervisors approved Contract #27-420-7 with Monica L. Wood (dba Pinnacle Mental Wellness Group) for the provision of outpatient psychotherapy services for the period of January 1, 2018 through December 31, 2019. The purpose of this board order is to reflect the correct name of the contractor to read as Pinnacle Mental Wellness Group instead of Monica L. Wood (dba Pinnacle Mental Wellness Group).

CONSEQUENCE OF NEGATIVE ACTION:

If this correction is not approved, the contractor’s legal status will remain incorrect.
