RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County, Contract #27-876-3 with S. Ming Chang, M.D., an individual, in an amount not to exceed $225,000 to provide pediatric primary care services to Contra Costa Health Plan (CCHP) members for the period June 1, 2018 through May 31, 2020.

FISCAL IMPACT:

This contract is funded 100% by CCHP Enterprise Fund II. (No rate increase)

BACKGROUND:

On May 24, 2016, the Board of Supervisors approved Contract #27-876-2 with S. Ming Chang, M.D., for the provision of pediatric primary care services to CCHP members, for the period from June 1, 2016 through May 31, 2018.

BACKGROUND: (CONT'D)

Approval of Contract #27-876-3 will allow the contractor to continue to provide pediatric primary care services for CCHP members through May 31, 2020.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialty health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the County will not be provided.
