RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Amendment Agreement #28-602-17, (State #13-20522 A-01) with the California Department of Public Health, effective May 1, 2017, to increase the amount payable to County by $117,983, to new payment limit of $3,364,091, for the Nutrition Education and Obesity Prevention Program, with no change in the original term of October 1, 2016 through September 30, 2019.

FISCAL IMPACT:

Approval of this amendment agreement will increase the amount payable to the County by $117,983 for the County’s Nutrition and Physical Activity Promotion Project. No County match required.


BACKGROUND:

On July 12, 2016, the Board of Supervisors approved Standard Agreement #28-602-16 with the California Department of Public Health, for the California Nutrition Network” Project for the period from October 1, 2016 through September 30, 2019. This Agreement provides nutrition education to Contra Costa County residents. The goal of the project is to educate the public, particularly low-income consumers, on healthful nutrition and physical activity practices to reduce risk for chronic disease.  
  
Approval of Amendment Agreement #28-602-17 will allow additional support to the Supplement Nutrition Assistance Program-Education (SNAP-Ed) eligible consumers by adopting healthy eating and physical activity behaviors, as part of a healthy lifestyle, through September 30, 2019.

CONSEQUENCE OF NEGATIVE ACTION:

If this amendment is not approved, the County will not receive funds to continue to educate SNAP-Ed eligible on healthful nutrition and physical activity practices, to help reduce risk for chronic disease.
