RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to transmit an application along with the necessary certification and assurances, to Contra Costa County Department of Conservation and Development (DCD), for State of California Emergency Solutions Grant (ESG) funding, in the amount of $60,000, to operate the County’s Coordinated Outreach, Referral and Engagement (CORE) Program for the period from October 1, 2017 through September 30, 2018.

FISCAL IMPACT:

The funds requested of the Contra Costa County DCD are essential to providing adequate outreach and engagement to homeless individuals, transition-aged youth, and families living outside in locations not meant for human habitation. County General funds committed to the operation of the CORE outreach program will be used to fulfill the 100% match requirement. No additional appropriation is needed.








BACKGROUND:

The Health Services Department Homeless Program is seeking funding to operate the County’s CORE outreach program. These services are aimed at identifying unsheltered homeless individuals, transition-age youth, and families living outside, and in locations not meant for human habitation. Outreach and engagement services include and are not limited to, providing linkages to benefits, MediCal enrollment, health and mental health access, completion of the Vulnerability Index-Service Prioritization Decision Assistance Tool (VI-SPDAT), access CARE centers, and interim housing.  
  
Approval will allow the County to receive State ESG funding from the Contra Costa Department of Conservation and Development, for continuous services and operation of the County’s CORE program, and to perform all responsibilities in relation to receipt of the funding for the period of October 1, 2017 through September 30, 2018.

CONSEQUENCE OF NEGATIVE ACTION:

If not approved, Health Services Department will not receive the additional funds to support the County’s CORE program and without such funding, may have to operate at a reduced capacity.
