RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract #26-771-2 with Health Leads, Inc., a non-profit corporation, in an amount not to exceed $1,475,352, for the provision of referral and outreach services to Contra Costa Regional Medical Center and Health Centers (CCRMC) patients requiring assistance for access to non-medical community resources, for the period from June 1, 2017 through May 31, 2020.

FISCAL IMPACT:

This Contract is funded 79% Hospital Enterprise Fund I and 21% Medi-Cal 1115 Waiver. (Rate Increase)

BACKGROUND:

On July 29, 2014, the Board of Supervisors approved Contract #26-771, (as amended by Amendment Agreement #26-771-1) with Health Leads, Inc., for the provision of data gathering of patients needing assistance in accessing food, childcare, energy bill assistance, and providing residential placement services for the period from June 1, 2014 through May 31, 2017.

BACKGROUND: (CONT'D)

Approval of Contract #26-771-2 will allow the Contractor to provide referral and outreach services to CCRMC patients requiring assistance for access to non-medical community resources, through May 31, 2020.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, CCRMC patients will not have access to Contractor’s services.
