Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Amendment/Extension Agreement #28–832-3 with Antioch Unified School District (AUSD), a government agency, effective March 1, 2017, to amend Interagency Agreement #28-832-2, to provide additional school-based mobile clinic services and extend the term from August 14, 2017 through August 31, 2020.

FISCAL IMPACT:

This is a non-financial agreement.

BACKGROUND:

This Contract meets the social needs of County’s population by providing mobile clinic services, including but not limited to: comprehensive physical exams, immunizations, tuberculin skin testing, sports physicals, reproductive health services and referrals, and well-child care to low-income and disadvantaged school children within AUSD District.

BACKGROUND: (CONT'D)

On October 7, 2014, the Board of Supervisors approved Interagency Agreement #28-832-2 with AUSD for the period from August 15, 2014 through August 14, 2017, allowing County to provide school-based mobile clinic services.

Approval of Contract Amendment/Extension Agreement #28–832-3, will allow County to continue to provide school-based mobile clinic services to children within the AUSD, through August 31, 2020.

CONSEQUENCE OF NEGATIVE ACTION:

If this amendment is not approved, Contractor will not continue providing mobile clinic services to children.

CHILDREN'S IMPACT STATEMENT:

This program supports the following Board of Supervisors’ community outcomes: “Children Ready For and Succeeding in School” and “Communities that are Safe and Provide a High Quality of Life for Children and Families”. Expected program outcomes include an increase in the number of healthy children within the District.
