RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County, Contract #27-940-1 with Claudell Stephens, M.D. & Yvonne Cobbs, NP, Medical Practices Healthy Living Clinic, a partnership, in an amount not to exceed $125,000 to provide primary care services for Contra Costa Health Plan members for the period from July 1, 2016 through June 30, 2018.

FISCAL IMPACT:

This Contract is funded 100% Contra Costa Health Plan Enterprise II Funds. (No rate increase)

BACKGROUND:

In August 2014, the County Administrator approved and the Purchasing Services Manager executed Contract #27-940 with Healthy Living Clinic for the provision of primary care services, for the period from July 1, 2014 through June 30, 2016. Approval of Contract #27-940-1 will allow Contractor to continue providing primary care services to Contra Costa Health Plan members through June 30, 2018.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialized health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the County will not be provided.
