RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County, Contract Amendment Agreement #27-253-12 with California Eye Clinic, a general partnership, effective June 1, 2016, to amend Contract #27-253-11, to increase the payment limit by $300,000, from $650,000 to a new payment limit of $950,000, with no change in the original term of May 1, 2015 through April 30, 2017.

FISCAL IMPACT:

This amendment is funded 100% Contra Costa Health Plan Enterprise Fund II. This amendment includes a new rate for ambulatory surgery services. (Rate increase)

BACKGROUND:

On April 21, 2015, the Board of Supervisors approved Contract #27-253-11 with California Eye Clinic for the provision of ophthalmology and optometry services for Contra Costa Health Plan members, for the period from May 1, 2015 through April 30, 2017. Approval of Contract Amendment Agreement #27-253-12 will allow the Contractor to provide Ambulatory Surgery Center services in addition to ophthalmology and optometry services through April 30, 2017.



CONSEQUENCE OF NEGATIVE ACTION:

If this amendment is not approved, patients will not have access to ambulatory surgery services provided by the Contractor.
