RECOMMENDATION(S):

Acknowledge receipt of notice from East Bay Perinatal Medical Associates, a Partnership, requesting termination of Contract #27-297-9 and ratify the agreement reached between the contractor and the Health Services Director to terminate the contractual obligations effective at the close of business on December 31, 2015.

FISCAL IMPACT:

This Contract is funded 100% Contra Costa Health Plan Enterprise Fund II

BACKGROUND:

On August 18, 2015, the Board of Supervisors approved Contract #27-297-9 with East Bay Perinatal Medical Associates for the provision of Maternal-Fetal Medicine services for Contra Costa Health Plan members, for the period from July 1, 2015 through June 30, 2017.

The purpose of this Board Order is to advise the Board of Supervisors that the Department and the Contractor have agreed to terminate Contract #27-297-9, effective at the close of business on December 31, 2015.
