RECOMMENDATION(S):

ADOPT Position Adjustment Resolution No. 21685 two (2) Community Health Worker II (VKVB) positions at salary level QT5-1043 ($36,422-$44,271); two (2) Registered Nurse-Experienced Level (VWXD) positions at salary level L3H-0400 ($87,667-$97,712); one (1) Public Health Program Specialist I (VBSD) position at salary level ZA5-1602 ($63,349-$77,001); three (3) Public Health Nurse (VVXA) position at salary level QT2-1832 ($83,457-$112,241) in the Health Services Department. (Represented).

FISCAL IMPACT:

Upon approval, this action has an annual cost of approximately $1,133,741.00, which includes $247,681.09 in pension costs. The cost will be funded by CCHP member ACA premiums and MAA/TCM revenue.









BACKGROUND:

Health Services is currently working across the multiple divisions in our health delivery system to determine and secure the staffing needs necessary to launch the high acuity health home at the West Counth Health Center (WCHC) by July, 2015. The staffing requirements are substantial to support the high-touch and high-acuity care that this health home will need to be successful. All divisions are committed to provide coordinated services including primary care in an interdisciplinary and integrated model with a high degree of access for patients enrolled including daily clinic accessibility for any patient in the cohort.  
  
We propose to develop a high acuity health home model for our super utilizer population at WCHC. The team will be interdisciplinary with a coordinated case management model that works to address both the medical and non-medical needs of the patients enrolled. The intervention team will coordinate and integrate multidisciplinary continuity services to address the complex, diverse and unmet needs that are not effectively addressed in high-cost acute care settings. The program design will follow some of the best practices and models developed around the nation with regard to caring for the patients with complex care. Most of these models rely on robust interdisciplinary team with coordinated case management services at the core to prioritize the diversity and acuity of services needed to care for this medically and socially complex population. We are borrowing from these models to design a specific and relevant model for our unique patients and system. The model is designed to address the complex and numerous physical, mental and social health needs amongst a specified high-risk, high-need, high-cost population. These interventions will provide appropriate medical, mental and social health needs therefore reducing cost, (both inappropriate and overall cost).  
  

CONSEQUENCE OF NEGATIVE ACTION:

If this action is not approved, staffing needs to launch the high acuity health home at West County Health Center by July 2015 will not be realized
