RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract Amendment/Extension Agreement #26-771-4 with Health Leads, Inc., a non-profit corporation, effective May 31, 2020, to amend Contract #26-771-2 (as amended by Amendment Agreement #26-771-3), to increase the payment limit by $278,515, from $1,475,352, to a new payment limit of $1,753,867, and extend the termination date from May 31, 2020 to December 31,2020, for additional referral and outreach services.

FISCAL IMPACT:

This Contract is funded by 70% Hospital Enterprise Fund I and 30% Medi-Cal 1115 Waiver. (No rate increase)

BACKGROUND:

On June 6, 2017, the Board of Supervisors approved Contract #26-771-2 (as amended by Amendment Agreement #26-771-3), with Health Leads, Inc. for the provision of referral and outreach services to Contra Costa Regional Medical Center (CCRMC) and Health Centers patients requiring assistance for access to non-medical community resources, for the period from June 1, 2017 through May 31, 2020.

BACKGROUND: (CONT'D)

Approval of Contract Amendment/Extension Agreement #26-771-4 will allow the Contractor to continue providing services through December 31, 2020.

CONSEQUENCE OF NEGATIVE ACTION:

If this amendment is not approved, Contractor will not continue providing services.
