RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment/Extension Agreement #26-744-2 with the Regents of the University of California, on behalf of its University of California, San Francisco School of Medicine, an educational institution, effective May 1, 2016, to amend Contract #26-744, with no change in the original payment limit of $105,000, and extend the termination date from June 30, 2016 through June 30, 2019.

FISCAL IMPACT:

This amendment is funded 100% Enterprise I Funds. (No rate increase)

BACKGROUND:

On August 6, 2013, the Board of Supervisors approved Contract #26-744 (as amended by contract amendment #26-744-1) with the Regents of the University of California, on behalf of its University of California, San Francisco School of Medicine, for the period from May 1, 2013 through June 30, 2016, for the provision of a residency training program in family medicine through June 30, 2016,

Approval of Contract Amendment/Extension Agreement #26-744-2 will allow the Contractor to continue providing services through June 30, 2019 including mutual indemnification to hold harmless both parties for any claims arising out of performance of this Agreement.

CONSEQUENCE OF NEGATIVE ACTION:

If this amendment is not approved, Contractor will not continue providing services.
