RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute on behalf of the County, Grant Agreement #28-847-1 with the Lucile Packard Foundation for Children’s Health, to pay County an amount not to exceed $25,000, for the Contra Costa California Community Care Coordination Collaborative (7 C’s) Project, for the period from January 1, 2015 through June 30, 2016.

FISCAL IMPACT:

This Grant Agreement will result in $25,000 from the Lucile Packard Foundation for Children’s Health for implementation of the 7 C’s Project. No County match.  









BACKGROUND:

The 7 C’s Project will build on the Early Childhood Leadership Alliance, a First 5 Initiative, focused on improving developmental and health outcomes for Contra Costa County children and families age birth to 5 years. The Project goals include: (1) Determining the specific needs for children (age birth to 5 years) with special health care needs and their families; (2) Aligning the organizational structure with providers and communities for Project implementation; (3) Conducting a three month pilot program prior to work through and validate procedural tools, costs and procedures prior to implementation; and (4) Creating and implementing a financial sustainability plan to secure care coordination staff and other resources.  
  
Approval of this Grant Agreement #28-847-1 will allow County continuous funding to support the implementation of the 7 C’s Project through June 30, 2016, including agreeing to indemnify and hold harmless the grantor for claims arising out of County’s performance under this Contract
