RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County as follows:  
  

1. Cancellation Agreement #76-524-12 with Samir B. Shah, M.D., Inc., (Specialty: Ophthalmology), a Professional Corporation, effective on the close of business on April 30, 2015; and

2. Contract #76-524-13 with Samir B. Shah, M.D., Inc., (Specialty: Ophthalmology), A Professional Corporation, in an amount not to exceed $1,875,000, for the provision of Ophthalmology services for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers, for the period from May 1, 2015 through April 30, 2018.  

FISCAL IMPACT:

This Contract is 100% funded by Hospital Enterprise Fund I. (Rate increase)

BACKGROUND:

On June 13, 2013, the Board of Supervisors approved Contract #76-524-11 with Samir B. Shah, M.D., Inc., for the period from June 1, 2013 through May 31, 2016, for the provision of ophthalmology services, including, consultation, clinic coverage, training, on-call services and medical and/or surgical procedures, and upon request Department Head of the assigned medical unit and Section Chief at Contra Costa Regional Medical Center and Contra Costa Health Centers.  
  
The Department and the Contractor have renegotiated the Contract terms for the Contractor to serve as Chief Medical Officer under the Chief Executive Officer at Contra Costa Regional Medical Center, upon request. Therefore, in accordance with General Conditions Paragraph 5. (Termination and Cancellation) of the Contract, the Department and Contractor have agreed to cancel Contract #76-524-11 and enter into a new Contract including the Chief Medical Officer services.   
  
Approval of Contract #76-524-13 will allow the Contractor to continue to provide additional Ophthalmology services and to act as Chief Medical Officer at Contra Costa Regional Medical Center and Contra Costa Health Centers, through April 30, 2018.  

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, patients requiring ophthalmology services at Contra Costa Regional Medical and Contra Costa Health Centers will not have access to Contractor’s services, which will result in a reduction in the overall level of services to the community.
