RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #22-806-23 with Public Health Foundation Enterprises, Inc., a non-profit corporation, effective April 1, 2015, to amend Contract #22-806-22, to increase the payment limit by $126,360, from $514,236 to a new payment limit of $640,596, with no change in the original term of July 1, 2014 through June 30, 2015.

FISCAL IMPACT:

This amendment is funded 85% by Federal Bioterrorism Grant, 3% by State AIDS Program Funds and 12% County Clinic Services funds. (Rate increase)

BACKGROUND:

On July 29, 2014, the Board of Supervisors approved Contract #22-806-22 with Public Health Foundation Enterprises, Inc., for the period from July 1, 2014 through June 30, 2015, for the provision of consultation and technical assistance on community health promotion for the County’s Community Education and Information Unit and the Emergency Medical Services Unit.

Approval of Contract Amendment Agreement #22-806-23 will allow the Contractor to provide additional services through June 30, 2015.  

CONSEQUENCE OF NEGATIVE ACTION:

If this amendment is not approved, County residents who depend on services from the Public Health Emergency Unit and the Community Education and Information Unit may not receive the services they need.
