RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County Contract #77-282-1 with Continuum Care Hospice LLC (dba Continuum Hospice), a limited liability company, in an amount not to exceed $390,000, to provide hospice and palliative care services to Contra Costa Health Plan (CCHP) members and County recipients for the period May 1, 2023 through April 30, 2026.

FISCAL IMPACT:

This contract will result in contractual service expenditures of up to $390,000 over a 3-year period and will be funded 100% by CCHP Enterprise Fund II revenues. (No rate increase)

BACKGROUND:

CCHP has an obligation to provide certain specialized hospice and palliative care health care services under the terms of their Individual and Group Health Plan membership contracts with the County. This contractor has been part of the CCHP Provider Network providing these services since May 1, 2020.

In March 2020, the County Administrator approved and the Purchasing Services Manager executed Contract #77-282 with Continuum Care Hospice LLC, in an amount not to exceed $100,000, for the provision of hospice and palliative care services for CCHP members and County recipients, for the period from May 1, 2020 through April 30, 2023.

Approval of Contract #77-282-1 will allow the contractor to continue providing hospice and palliative care services through April 30, 2026.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialized hospice and palliative care services for CCHP members under the terms of their Individual and Group Health Plan membership contract with the County will not be provided.
