RECOMMENDATION(S):
APPROVE and AUTHORIZE the Health Services Director, or designee, to execute on behalf of the County, as follows: (1) Cancellation Agreement #27-735-10 with David Gilbert, M.D., an individual, effective at the end of business on April 30, 2022; and (2) Contract #27-735-11 with David D. Gilbert, MD, Inc. (dba Diablo Eye Associates), a professional corporation, in an amount not to exceed $300,000, to provide ophthalmology services for Contra Costa Health Plan (CCHP) members, for the period from May 1, 2022 through April 30, 2025.
FISCAL IMPACT:
This contract will result in contractual service expenditures of up to $300,000 over a three-year period and will be funded 100% by CCHP Enterprise Fund II revenues. (No rate increase)
