RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Cancellation Agreement #26-506-5 with East Bay Perinatal Medical Associates, a general partnership, effective at the close of business on December 31, 2015.

FISCAL IMPACT:

This Contract is funded 100% Hospital Enterprise Fund I.

BACKGROUND:

On February 11, 2014, the Board of Supervisors approved Contract #26-506-4 with East Bay Perinatal Medical Associates for the provision of perinatology services, for the period from January 1, 2014 through December 31, 2016. In accordance with General Conditions Paragraph 5 (Termination), of the Contract, the Department and Contractor have agreed to a mutual cancellation of this Contract. Approval of Cancellation Agreement #26-506-5 will accomplish this termination, effective at the close of business on December 31, 2015.
