RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee to accept, on behalf of the County, Grant Award #28-872 from the March of Dimes Foundation – California Chapter, in an amount not to exceed $3,000, for the Nurse-Family Partnership Program (NFP), Inter-conception Care Events 2015, for the period from January 1, 2015 through December 31, 2015.

FISCAL IMPACT:

Acceptance of this grant will result in an amount payable to the County not to exceed $3,000 from the March of Dimes Foundation, California Chapter for the NFP Program, Inter-conception Care and Graduation Events 2015. (No County match)

BACKGROUND:

The County’s Public Health Family, Maternal & Child Health Programs in collaboration with Children & Family Services, Perinatal Home Visiting Programs, Community Advisory Board and other participating community service providers will work together to provide NFP’s graduating clients and participants with educational resources related to inter-conception care. The goal of the NFP Program is to educate graduating clients and community members on the importance and benefit of inter-conception care; and to encourage the clients to remain confident in their abilities to positively shape their children’s lives; and to celebrate and acknowledge each client’s investment of time and effort to better their families’ lives. The participants will complete post-surveys which will indicate if NFP should adjust its delivery methods, and whether community events are effective platforms for delivering health education.

CONSEQUENCE OF NEGATIVE ACTION:

If this grant is not approved, the County will not receive funds for the Inter-conception Care Events 2015 Program.
