RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee (Jeff Smith, M.D.), to execute on behalf of the County, Cancellation Agreement #26-580-3 with Akindele Kolade, M.D., (Medical Specialty: Psychiatric services), a self-employed individual, effective at the close of business on April 30, 2009.

FISCAL IMPACT:

This Contract is funded 100% by Enterprise Fund I.

BACKGROUND:

On January 8, 2008, the Board of Supervisors approved Contract #26-580-2 with Akindele Kolade, M.D.,
for the period from January 1, 2008 through December 31, 2010, for the provision of psychiatric emergency services for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers.

BACKGROUND: (CONT'D)

In accordance with General Conditions Paragraph 5 (Termination and Cancellation), of the Contract, the Department and Contractor have agreed to a mutual cancellation of this Contract. Approval of Cancellation Agreement #26-580-3 will accomplish this termination.
