RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract #27-830-3 with Pleasanton Physical Therapy Services, Inc. (dba Back on Track Physical Therapy), a corporation, in an amount not to exceed $500,000, to provide physical therapy services for Contra Costa Health Plan members for the period from February 1, 2017 through January 31, 2019.

FISCAL IMPACT:

This contract is funded 100% by Contra Costa Health Plan Enterprise Fund II.

BACKGROUND:

On April 14, 2015, the Board of Supervisors approved Contract #27-830-2 with Pleasanton Physical Therapy Services, Inc., (dba Back on Track Physical Therapy) for the provision of physical therapy services for Contra Costa Health Plan members, for the period from February 1, 2015 through January 31, 2017. Approval of Contract #27-830-3 will allow Contractor to continue providing physical therapy services through January 31, 2019.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialized health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the County will not be provided.
