RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract #77-009 with H K Facial Plastic Surgery, Inc., a corporation, in an amount not to exceed $140,000, to provide plastic and reconstructive surgery services to Contra Costa Health Plan (CCHP) members, for the period from March 1, 2016 through February 28, 2018.

FISCAL IMPACT:

This Contract is funded 100% Contra Costa Health Plan Enterprise Fund II.

BACKGROUND:

The Health Plan has an obligation to provide certain specialized health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the County. Under Contract #77-009, the Contractor will provide plastic and reconstructive surgery services through February 28, 2018.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialized health care services for its members under the terms of their Individual and Group Health plan membership contracts with the County will not be provided.
