RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Cancellation Agreement #26-751-1 with Jill Antoine, M.D., a self-employed individual, effective at the close of business on November 30, 2013

FISCAL IMPACT:

This Contract is funded 100% by Enterprise Fund I.

BACKGROUND:

On September 17, 2013, the Board of Supervisors approved Contract #26-751 with Jill Antoine, M.D. for the provision of professional anesthesiology services at Contra Costa Regional Medical Center (CCRMC), for the period from August 1, 2013 through July 31, 2014.

BACKGROUND: (CONT'D)

In accordance with General Conditions Paragraph 5 (Termination), of the Contract, the Department and Contractor have agreed to a mutual cancellation of this Contract. Approval of Cancellation Agreement #26-751-1 will accomplish this termination.

CONSEQUENCE OF NEGATIVE ACTION:
