Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #24-393-39(14) with Kiran U. Koka, M.D., A Professional Corporation, effective April 1, 2013, to amend Contract #24-939-39(13), to increase the payment limit by $40,000, from $150,000 to a new payment limit of $190,000, with no change in the original term of July 1, 2011 through June 30, 2013.

FISCAL IMPACT:

This amendment is funded 100% by MediCal Funds offset 50% Federal, 50% State (No rate increase)

BACKGROUND:

On June 21, 2011, the Board of Supervisors approved Contract #24-939-39(13) with Kiran U. Koka, M.D., A Professional Corporation, for the provision of Medi-Cal specialty mental health services, for the period from July 1, 2011 through June 30, 2013.
