RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee (Jeff Smith, M.D.) to accept on behalf of the County Grant Award #29-677 (08-1057) from the California Healthcare Foundation, to pay the County an amount not to exceed $40,000 for the Enabling Chronic Disease Registries Project, a Multi-Condition Chronic Disease Management System (CDMS), for the period from May 1, 2008 through February 28, 2009.

FISCAL IMPACT:

Approval of this Award will result in $40,000 from the California Healthcare Foundation for the County’s Regional Medical Center Enabling Chronic Disease Registries Project, a Multi-Condition Chronic Disease Management System (CDMS). No County funds are required.











BACKGROUND:

Fourteen million Californians have at least one chronic condition, while seven million have two or more conditions and yet direct health care accounts for more than 75 percent of total health care expenditures. Despite a wealth of scientific evidence and the availability of information about best practices in chronic disease care, many patients do not receive the care and support they need to effectively manage their conditions. Funding for this Enabling Chronic Disease Registries Project, a Multi-Condition Chronic Disease Management System (CDMS), will allow the County’s Regional Medical Center to purchase this chronic disease management system with the proper functionality to accelerate the spread of chronic disease improvement work throughout Contra Costa County. The goal of the project will be to improve health outcomes and assist in the reduction of health disparities, while having the ability to track and monitor the care of patients with chronic conditions over time, identify those with gaps in care and reach out to bring them in for needed tests or treatment, and to provide clinicians with timely and accurate information about what is needed at the time of a patient visit.   
  
Approval of Grant Award #29-677 will provide funding to expand the chronic disease registry functionality and implementation of a new chronic disease management system (CDMS) at Contra Costa Regional Medical Center through, February 28, 2009.
