RECOMMENDATION(S):

Acknowledge receipt of notice from John Chan, M.D., a self-employed individual, requesting termination of Contract #26-521-2, for the provision of anesthesiology services at Contra Costa Regional Medical Center and Contra Costa Health Centers, effective at the end of the workday on January 3, 2009.

FISCAL IMPACT:

This Contract was funded 100% by Enterprise Fund I.

BACKGROUND:

On April 15, 2008, the Board of Supervisors approved Contract #26-521-2 with John Chan, M.D., for the period from May 1, 2008 through April 30, 2011, for the provision of professional anesthesiology services for patients at Contra Costa Regional Medical Center and Contra Costa Health Centers.

The purpose of this Board Order is to advise the Board of Supervisors that the Department and the Contractor have mutually agreed to terminate Contract #26-521-2 with John Chan, M.D., in accordance with Special Conditions, Paragraph 2. (Termination and Cancellation) of the Contract.
