RECOMMENDATION(S):

APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, on behalf of its Aging and Adult Services Bureau, to apply for and accept funding from the California Department of Health and Human Services, Housing Bureau in an amount not to exceed $1,382,181 for the Home Safe Program for the period of July 1, 2021 through June 30, 2024.

FISCAL IMPACT:

County to receive an amount not to exceed $1,382,181 from the California State Budget for Fiscal Year 2021-22 to be expended through June 30, 2024 to fund the Home Safe program over a three-year period. Match requirement is exempt.

BACKGROUND:

The goal of the Home Safe Program (Home Safe) is to support the safety and housing stability of individuals involved in Adult Protective Services (APS) by providing housing-related assistance using evidence-based practices for homeless assistance and prevention. Home Safe will serve APS clients that are experiencing homelessness or are at imminent risk of homelessness due to elder or dependent adult abuse, neglect, self-neglect, or financial exploitation and was expanded to include individuals who are in the intake process for APS. Clients must also voluntarily agree to participate in the program.

Support short-term housing interventions including case management paired with one-time, short-term or shared housing assistance consistent with evidence-based practices; Address program gaps and build capacity in areas needed to support partnerships between APS, Coordinated Entry System and the local homelessness system; and Strengthen linkages & working relationships among agency partners working with APS clients to better meet the needs of vulnerable and homeless older adults at risk of homelessness.
The department's Aging & Adult Services Bureau will partner with Contra Costa Health Services, Health, Housing & Homelessness Services (H3), and contracted partners to deliver the housing prevention and interventions outlined in the grant proposal application. Additional partners may be identified and added as needed.

Without funding, the APS Program will continue to face an increasing number of vulnerable older adult clients becoming homeless without prevention or diversion activities to mitigate the rising costs of affordable housing, loss of housing subsidies, and lack of appropriate short-term housing options.
