RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract Amendment Agreement #27-976-2 with Diablo Medical and Sleep Clinic Medical Corporation, a corporation, effective November 1, 2017, to amend Contract #27-976, to increase the payment limit by $105,000, from $120,000 to a new payment limit of $225,000, to provide additional pulmonary and sleep medicine services to Contra Costa Health Plan members with no change in the original term of June 1, 2015 through May 31, 2018.

FISCAL IMPACT:

This amendment is funded 100% by Contra Costa health Plan Enterprise Fund III. (Rate increase)

BACKGROUND:

On August 18, 2015, the Board of Supervisors approved Contract #27-976 with Diablo Medical and Sleep Clinic Medical Corporation for the provision of pulmonary and sleep medicine services for Contra Costa Health Plan members, for the period from June 1, 2015 through May 31, 2018.

BACKGROUND: (CONT'D)

Approval of Contract Amendment Agreement #27-976-2 will allow the contractor to provide additional pulmonary and sleep medicine services through May 31, 2018.

CONSEQUENCE OF NEGATIVE ACTION:

If this amendment is not approved, certain specialized health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the County will not be provided.
