RECOMMENDATION(S):

APPROVE and AUTHORIZE the Health Services Director, or designee, to terminate contract #27-988-1, with Amanda Ibarra Ranola, OD (dba Optometric Vision Care) for optometry services for Contra Costa Health Plan members, effective close of business on October 27, 2017, as agreed upon by both parties.

FISCAL IMPACT:

This contract was funded 100% by Contra Costa Health Plan Enterprise Fund II.

BACKGROUND:

On August 8, 2017, the Board of Supervisors approved Contract #27-988-1 with Amanda Ibarra Ranola, OD (dba Optometric Vision Care) for the provision of optometry services for Contra Costa Health Plan members, for the period from September 1, 2017 through August 31, 2019.

The Department received a letter from the contractor, requesting termination of this contract. In accordance with General Conditions, Paragraph 5 (Termination), the contractor served the Department 30 days written notice of cancellation of this contract.
