RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Contract #77-049 with LTP Care Continuum, Corp., a corporation, in an amount not to exceed $150,000, to provide skilled nursing services to Contra Costa Health Plan (CCHP) members, for the period from January 1, 2017 through December 31, 2017.

FISCAL IMPACT:

This Contract is funded 100% Contra Costa Health Plan Enterprise Fund II.

BACKGROUND:

The Health Plan has an obligation to provide certain specialized health care services for its members under the terms of their Individual and Group Health Plan membership contracts with the County. Under Contract #77-049, the Contractor will provide skilled nursing services to CCHP members through December 31, 2017.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, certain specialized health care services for Contra Costa Health Plan members under the terms of their Individual and Group Health plan membership contracts with the County will not be provided by this contractor.
