RECOMMENDATION(S):

Approve and authorize the Health Services Director, or designee, to execute on behalf of the County, Contract #27-162-7 with Juan R. Sequeira, M.D., an individual, in an amount not to exceed $300,000, to provide primary care physician services to Contra Costa Health Plan (CCHP) members and County recipients for the period from December 1, 2016 through November 30, 2018.

FISCAL IMPACT:

This Contract is funded 100% Contra Costa Health Plan Enterprise Fund II. (No rate increase)

BACKGROUND:

On December 16, 2014, the Board of Supervisors approved Contract #27-162-6 with Juan R. Sequeira, M.D., for the provision of primary care services to Contra Costa Health Plan members and County recipients, for the period from December 1, 2014 through November 30, 2016. Approval of Contract #27-162-7 will allow the Contractor to continue providing primary care physician services through November 30, 2018.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, CCHP and County recipients would not have access to Contractor’s services, which may result in a reduction in the overall levels or service to the community.
