RECOMMENDATION(S):

Approve and authorize the Health Services Director, or designee, to execute on behalf of the County, Contract #27-188-7 with Gretchen D. Graves (dba Gretchen D. Graves, M.D.), an individual, in an amount not to exceed $500,000, to provide pediatric primary care services for Contra Costa Health Plan (CCHP) members and County recipients, for the period from December 1, 2016 through November 30, 2018.

FISCAL IMPACT:

This Contract is funded 100% Contra Costa Health Plan Enterprise Fund II. (No rate increase)

BACKGROUND:

On December 2, 2014, the Board of Supervisors approved Contract #27-188-6 with Gretchen D. Graves, M.D., for the period from December 1, 2014 through November 30, 2016, to provide pediatric primary care services to CCHP members and County recipients. Approval of Contract #27-188-7 will allow the Contractor to continue to provide pediatric primary care services through November 30, 2018.

CONSEQUENCE OF NEGATIVE ACTION:

If this contract is not approved, Contra Costa Health Plan members and county recipients would not have access to Contractor’s services.
