RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee, to execute, on behalf of the County, Amendment Agreement #28-702-13 (#12-024-0) with First 5 Contra Costa Children and Families Commission, effective October 1, 2012, to increase the total payment limit payable to County, by $133,607, from $430,350 to a new total payment limit of $563,957, and to extend the term from December 3, 2012 through June 30, 2013.

FISCAL IMPACT:

This amendment agreement will result in an increase of $133,607 from First 5 Contra Costa Children and Families Commission for County’s Home Visiting Program through June 30, 2013. No County match required.

BACKGROUND:

First 5 Contra Costa Children and Families Commission has adopted a Home Visiting strategy which has been developed through extensive collaboration and planning with representatives of nine home visiting programs in Contra Costa County. The goal of the Home Visiting Program plan is to assure that children in our communities, ages 0-5, are healthy and have access to quality preventive well-child primary care.

On November 8, 2011, the Board of Supervisors approved Agreement #28–702–11 (as amended by Amendment Agreement #28-702-12) with First 5 Contra Costa Children and Families Commission, for the period from July 1, 2011 through December 31, 2012.  
  
Approval of Amendment Agreement #28–702–13 will allow additional funds to continue the Home Visiting Program through June 30, 2013.  

CONSEQUENCE OF NEGATIVE ACTION:

If this amendment is not approved, County will not receive the additional funding to continue providing home visiting services to medically vulnerable infants and their families.
