RECOMMENDATION(S):

Adopt Position Adjustment Resolution No. 21208 to add one (1) Registered Nurse- Experienced Level (VWXD) position at salary level L3H 0400 ($7091.26 - $7903.79), in the Health Services Department

FISCAL IMPACT:

Upon approval, this action will result in an annual cost of approximately $138,223.80, including pension costs of $30,208.76, and will be offset by money saved from reducing preventative admissions, and member premiums. 100% Enterprise Fund III.

BACKGROUND:

The Health Services Department Contra Costa Health Plan (CCHP) is starting a new program that will help hospital discharged patients throughout the County get back into primary care more quickly. This program will help offset the financial losses currently incurred from the State by decreasing the number of Potentially Preventable Admissions. Therefore, a registered nurse position is needed to work with hospital social workers and discharge planners to coordinate care for patients triaged back to primary care or triaged to Case Management. This position will provide a level of assistance that will improve the transitional care of hospitalized patients, which can prevent unnecessary emergency room visits and readmissions.



CONSEQUENCE OF NEGATIVE ACTION:

Failure to approve this action will prevent the Contra Costa Health Plan’s Case Management unit from improving the transitional care of patients and saving the health plan costs associated with Potentially Preventable Admissions.
